Tri-County Retirement and Nursing Home (TCH) is a non-profit, corporation that operates a fifty-eight-bed skilled nursing facility and an eleven apartment assisted living facility. The Corporation is owned by seven area churches: Beaver Creek Lutheran, Bethany Lutheran, Goose River Lutheran, Holmes United Methodist, Little Forks Lutheran, St. John Lutheran, and Zoar Free Lutheran.
Physical, occupational and speech therapies are available to both nursing home and community residents. The staff works hard to provide high quality care for which Tri-County Retirement and Nursing Home is known for.
Tri-County Retirement and Nursing Home staff believe in the importance of caring for the needs of the whole individual, physically, emotionally and spiritually. This belief is the basis for all the services we provide and enables us to enrich the lives of our residents. The care we provide is personally designed to involve and enhance every aspect of life, creating an environment that offers tranquility as well as stimulating daily living. Our facility provides a full range of nursing home care for our elderly residents, including supervised activities, specialized care, and transportation to medical appointments, recreational and social activities, as well as meet their physical, emotional and spiritual needs.
The mission of the Tri-County Retirement & Nursing Home is to provide physical, social and spiritual care and support to the elderly for the residents of the Tri-County area.
Skilled Nursing Care:
A physician's order is required for admission, in addition to a medical and social history and a physical examination within 5 days. A pre-admission screening may be required to determine the need for skilled care.
TCH will admit only those individuals for whom all necessary services are available. TCH encourages pre-admission visits to allow the applicant and family to tour the facility and meet the staff. Contact the Social Service Dept. for an appointment.
Donations, Memorials & Trust Funds:
The TCH family conducts regular fund-raising events to ensure community involvement and continued viability. Donations and memorials may be applied to special projects and will be recognized. The TCH Foundation manages trust funds designated by individuals to be used to ensure continuation of services in a comfortable environment.
